HOW TO REFER A CLIENT

ntering treatment can be one of the most life-altering events a person can experience  Corner Canyon Health Centers is committed to providing each client, their family, and referring professionals with compassion, clinical expertise, and effective communication during the therapeutic admissions process.

In order to help you support your clients in their admission to Corner Canyon Health Centers please contact us at 877-717-6237 or info@cornercanyonhc.com. A team member will work hand-in-hand with the prospective client or family member during the entire admission process.

From first contact, an Admissions Coordinator will conduct an initial conversation with the caller to discuss the presenting issues. The resident will be asked to identify if he or she is working with an outside treatment provider, who will be contacted for clinical information. Initial information on Corner Canyon Health Centers will be shared. The Admissions Coordinator can also verify insurance benefits and discuss financial arrangement options.

A pre-admission interview, which takes about 20-30 minutes, will take place by phone to assess if Corner Canyon is truly appropriate for the prospective client. The Admissions Coordinator will also gather any pertinent clinical information from the outside treatment professional at this time. If the resident is appropriate for admission, arrangements (travel, logistics, etc.) are made.

If an interventionist is involved in making the referral, the pre-admission assessment may take place with a family member.

If a referring professional is having difficulty at all during this process, he or she is welcome to call the Outreach Professional or Admissions Coordinator for additional information.

 

WHEN A CLIENT HAS BEEN ADMITTED

The referring treatment professional will be called within 24 hours of a client’s admission to let him or her know of the client’s safe arrival. The client will be asked to sign a release for his or her outside treatment professional within the first 24 hours. Provided the client signs a Release for his or her outside treatment provider(s), the therapist will call with ongoing updates regarding client progress, medication changes and discharge planning (please note, if you do not receive communication from the Corner Canyon Health Centers treatment team, it’s likely due to the client declining to sign a Release). While we respect our clients’ rights for confidentiality, we consider not being able to communicate with the entire treatment team to be a clinical issue that will be utilized in therapeutic processing.

The Family Program is typically held every two to three weekends. The client’s therapist will contact family members after the client completes his or her Family Invitation List and signs releases (typically within the first week of treatment).

She moved to UT in 2013 and completed her bachelors degree in Psychology in 2016. After college Liz worked at a residential treatment center and found that she was not only passionate about people, but also administration. Liz is recently finished her MPA in April 2022.
